This original and powerful book illuminates a perennially controversial subject via detailed case histories of 54 children with schizophrenia. The book begins with a review of 130 years of the psychiatric literature on psychosis in childhood. Cantor then presents a detailed analysis of the developmental and family histories of the children in her study, comparing some of the data to a non-schizophrenic control group and yielding suggestive insights on the roots of vulnerability to mental illness.The difficulties involved in assessment and treatment are examined in depth, and useful diagnostic methods are discussed. Also presented are ideas and tools for monitoring change in the child's clinical state and evaluating the effectiveness of interventions.

CHOICE Review

Childhood schizophrenia is not yet recognized as a valid diagnosis according to the American Psychiatric Association. Yet in this book the author claims that the disorder is real and that diagnosis in childhood is critical for proper treatment of affected individuals. One of Cantor's goals is symptom clarification and the improvement of chances that schizophrenic children will be identified. She is going against the tide of professional opinion and is more optimistic than the APA. On the positive side, the book does a fine job of relating what is known about "vulnerable children." Case studies give a flavor of the range of odd behaviors, bizarre reasoning, and unusual social skills to be found. Research into possible causes and common symptoms is discussed; common diagnostic problems and potential therapies are also treated. On the negative side, terms such as hypotonia and brachycephaly are used without definition; presumably readers will be familiar with clinical terminology. Also, well-documented problems in diagnosis are never resolved (e.g., symptoms in adults that allow a diagnosis of schizophrenia are not evident in childhood). No one behavior is characteristic of even a simple majority of the children Cantor describes.
